Please use the attached spreadsheet to calculate the cost of cancellation for client's requesting to cancel their Care Companion Plan, or plans that need to be sent to collections.
Ensure you are using the correct tab on the spreadsheet for plan itemization. CCP version 1 is for hospitals who have the original version of wellness plans (if your hospital has the Premium Dental plan this is for you). Version 2 is for "WP" hospitals to use (if your hospital only offers one dental cleaning plan).
