Employee Care Companion Plan Benefit Details

Overview:

Full-time employees will receive two Care Companion Plans for their personal pets. Part-time employees are eligible for one Care Companion Plan. 

There are four Employee Care Companion Plan options to choose from:

  • Puppy Plan with optional spay/neuter 
  • Premium Dental Dog
  • Kitten Plan with optional spay/neuter 
  • Premium Dental Cat

There are no out of pocket expenses for included services/products as the employee plans are classified as in IRS fringe benefit.    

  • Employees will also receive 20% off any services not included in the Care Companion Plans.
  • Flea/tick and heartworm prevention are not included in the Employee plans. The flea/tick and heartworm products are eligible for the 20% employee discount.

Eligibility:

  • Employee is working for a hospital that uses Pulse and the hospital offers wellness plans. 
  • Employees must select from the currently available plans at their hospital. 
  • Employees are eligible on the first of the month following thirty (30) days of employment. 
  • Employees classified as Full Time (working 30+ hours per week) are eligible for up to two plans listed above. 
  • Employees classified as Part Time (regularly scheduled, not temp or per diem) are eligible for 1 plan listed above. 
  • Relief veterinarians are not eligible for employee Care Companion Plans.  

Details:

  • Care Companion Plans are considered a benefit; therefore, employees will see the plan(s) listed as a payroll fringe benefit on their paycheck stub after their enrollment. (There is no charge associated with this benefit to the employee).
  • Plans are good for 364 days from the date of enrollment. At the time of plan expiration, employees will need to re-enroll for the same pet or may opt to enroll a different pet.
  • Once enrolled, Employee Free plans are non-transferrable and can not be cancelled for any reason, even if the pet passes away.
  • If an employee separates from the company, whether voluntary or involuntarily, the free plans will be cancelled, and the employee discount no longer applies for any services or plans.

This employee CCP benefit offering, when enrolled through Pulse will require that any other prior employee discounts on services and products be cancelled and no longer offered or utilized.
